iTunes Gift Card "Code is Not Valid" !

I tried to use the iTunes Gift card and I get a response:

"The code entered is not valid"

Needless to say I have already spent enough time first figuring out how to "Redeem" a card - primarily due to the sparse directions Apple provides - and lastly - I have now tried various codes from the gift card - with not much success.

Note: iTunes Gift Cards are only Valid in Country of Issue.
