We are not Apple here - we are just users like you. Also, not a good idea to put out information about you or your devices on a public forum.
Take your phone back to the store that your purchased it from so they can take a look at it and determine if there is a hardware issue.
You can try to reset the phone (no data will be lost): Hold down the Home and Power buttons at the same time and continue to hold them down until the Silver Apple appears (up to 30 seconds). Once the Home screen redisplays, see if your issues are resolved.
Other than that, there is not a lot more we can do for you here, since you have already done a full restore. Take it to the Apple store to have it looked at.
