Every once in a while during startup on my early 2009 MacPro running Mavericks the system freezes and list an error code

I did a clean install and the problem still exists.

Suggestions?

Hi,

Most instances I see causing that are 3rd party Kext extensions being out of date or corrupted.

But Are you saying you erased & Installed, or just reinstalled the OS?

One way to test is to Safe Boot from the HD, (holding Shift key down at bootup), run Disk Utility in Applications>Utilities, then highlight your drive, click on Repair Permissions, Test for problem in Safe Mode...

PS. Safe boot may stay on the gray radian for a long time, let it go, it's trying to repair the Hard Drive

Reboot, test again.

If it only does it in Regular Boot, then it could be some hardware problem like Video card, (Quartz is turned off in Safe Mode), or Airport, or some USB or Firewire device, or 3rd party add-on, Check System Preferences>Accounts (Users & Groups in later OSX versions)>Login Items window to see if it or something relevant is listed.

Check the System Preferences>Other Row, for 3rd party Pref Panes.

Also look in these if they exist, some are invisible...

Code was copied via camera, then converted to PDF character reconition, then corrections made that Accrobat missed... just hope I caught them all.

I was having troubles upgrading to Mavericks anyway so my past experience has been that it's always best to do a clean install with a substantial OS upgrade. I also reinstalled all software as well.

I've wondered as well if it could possibly be a video card going bad as one of the things you suggested but I'd like to check out other areas first before making a big investment.

As this happens infrequently I'll be able to go through the suggested places to check first

That kind of panic seems to be caused by a fault in the graphics processor or another PCI device, by some other kind of hardware fault, or (perhaps) by some kernel extensions. If you can reproduce the panic in safe mode after reinstalling the OS, that rules out software as the cause. Remove all aftermarket PCI cards, if any, and test.
