Q: My iPhone 5 camera doesn't focus, please help.

I have had my iPhone 5 for about 15 months so warantee has expired. The camera has recently stopped focusing when in use. Items in clise up are crisp, but anything in mid range to long distance are blurred. The camera tries to focus, and 'zooms in and out' to do so, but then when it 'thinks' it is in focus, it is infact blurred.

I have cleaned the lense, tapped the phone, and restarted the phone.

Please help!

Any case around the phone? If yes, remove it and try again. Does the front camera focus correctly?
To rule out a software issue, set up the phone as new device, without using the latest backup afterwards.
If the camera works after that, restore from the backup. If it does not, you'll have to get the phone serviced:
