Since dropping the phone is not covered by the warranty, you'll have to pay for the device being replaced. Apple does not repair just parts of the iPhone, they will replace the whole unit.

Out-of-Warranty Service

If you own an iPhone that is ineligible for warranty service but is eligible for Out-of-Warranty (OOW) Service, Apple will service your iPhone for the Out-of-Warranty Service fee listed below.

Apple Store can replace iPhone 5 screens for $149.

iPhone 5, 4S, 4 can have back covers & batteries replaced in store.

Yes.

I could be wrong, but didn't Apple want to test that in a few Apple Stores in the US before deciding wether to do that in every Apple Store?

They did test it and implemented it for the models Chris mentioned.
