Q: Old Mac (Performa) Monitor Port to VGA adapter - how to make.

I have to start up my late father's Performa, but the monitor is dead.

Can you point me to a suitable connection diagram of a DB-15 to VGA adapter.

Try a general web search. You can probably find an adapter for less than it will cost in time and materials.
I did this

25 pin to vga pinout

and found multiple hits on vendors, also some with pinout images if you really want to go that way.
