I get no audio from this device. All my HDMI ports are in use with the cable box, DVR and receiver . My digital optical cable is also in use to the receiver. Is there a solution?

if all your hdmi ports are in use then you would not get both video and audio

How exactly is the ATV connected if all HDMI ports are in use?

If connected directly to the TV, then audio is being sent to the TV speakers, not the receiver.
