How can I use Image Capture to data-bend images?

I scanned an image from my HP OfficeJet 4500 (purchased with my MBP in late 2011). It looks like this (scaled down and saved as a jpg to upload here):

I used Image Capture, and know for sure that it was scanned at 300dpi as a png. I then opened it in Adobe Illustrator CS5 and it displayed as this:
