This seems to be a huge flaw. If you have a long list it seems to me that you have to enter new items at the bottom of the list and then move them up. Is this just me not understanding the application or is it a design flaw?
I don't know that it's a flaw just because it's not the way you want it. That appears to be just how the app is. That's not a bad idea, though. You could give Apple feedback here and suggest it: Apple - Mac OS X - Feedback
No, but you can click and hold an entry and drag it where you want it in the list.
