how do you unlock a computer when you don't know the password?

I have a cousin's Macbook whose computer has been locked and no one can remember the password to open it. How do you reset the password and open it up when you don't know the original password?

Depends on which OS X version you have. 10.6 and prior vs 10.7, 10.8, 10.9
