I've lost the voice who says playlist names, it just says "playlist 1"
I've lost the woman's voice who spoke the playlist, eg "gym". I get a male voice saying "playlist 2" or whatever

That feaure is called VoiceOver. It sounds like VoiceOver is disabled. The settings to enable VoiceOver are on the Shuffle's Summary screen in iTunes. This document provides information on enabling and using VoiceOver.
