Very bad outgoing voice quality with Bluetooth handsfree car sets

We cannot be the only people with this problem. We are with different carriers, different bluetooth hands free systems, but both have this problem of the outgoing voice quality being diabolical. We have both tried our units with other (blackberry) phones, and the problem is gone, so it's Iphone specific, please can anyone help!!!!
