Try an SMC reset. Open Activity Monitor in the Applications>Utilities Folder. Make sure All Processes are showing and not just My Processes and look at the CPU tab. See if something is using a lot of CPU causing an increase in heat resulting in the fans going full blast.

Fans back on at full bore. smc/pram will not reset it this time. Imac was working fine until update 10.9.2

Same here. I've reset pram, smc, checked for any CPU intensive apps, bought new battery, even opened case and blew dust out of fans. Was going to do a heat sink clean but another thread brought up the idea of Mavericks being the culprit. I hadn't thought of it before but I'm starting to think it has to do with Mavericks.
