The app is a free download from the App store.  It is used to track one lost/stolen iOS device from another iOS device.  On your iPad, simply launch the app, login with the same iCloud ID and password you used to set up find my iPhone on the lost iPhone, and hope that it shows up.

It will not show up if you did not enable find my iPhone under your iCloud account settings on the iPhone beforehand.  Nor will it show up if the lost iPhone is powered off or the battery had run down, or if it is unable to connect to the internet (wifi or cellular connection is fine, but it needs a live internet data connection to be trackable).

If this does not anwser your question (HT5818 is Apple's document about Activation Lock, so I'm not sure just what your question is), post back with specifics of what your problem is or what exactly you wish to do.
