Sound Plays mono when i plug headphones!

Hi

I just realise that Sound Plays mono when i plug headphones!

I tried many types of heaphones, always mono.

I tried every configuration in every way, still MONO!

I think it started when the latest version of Itunes 11.1.5 (5) download and install automatically

Any clue?
