What is my ipad 2 or 3 or 4. Model number is 1458. What does 4th generation mean?

The HT5452 page that you posted from shows it as being a 4th gen iPad (i.e. the fourth version). That was the model that was released in November 2012, the fifth (and current model) was released in November 2013)

There is officially no "iPad 4" nor "4th generation of iPad 2". Apple's official model designations are as follows:
iPad (aka "iPad 1" but not officially called that by Apple)
iPad 2
iPad 3rd generation
iPad 4th generation
iPad Air
This doesn't include the iPad minis.

There isn't an 'iPad 2 4th generaton', there is only one iPad 2

iPad 5 : November 2013
To avoid any potential confusion, those are not the official Apple designations that KP posted, though those designations are ones often used by users. Not by Apple, though, which uses the designations I gave above.
