I need to lock the 4S so to not butt dial, accidentally send messages etc when phone is in pocket, messenger bag. When I do this notifications, i.e calendar reminders are not audible. Do not disturb is not enabled. My only choice at bottom of DND screen in SILENCE area is 'ALways' or "only When IPhone is locked" How do I disable this?

You want Sounds not DND.
Settings > Sounds
Be sure your Mute isn't on (side switch by the volume) and your volume is up.
Take a look at Notification Center also:

I want a NEVER under the Silence area in DND. How do I get that? I need to lock the phone and when I do it will not allow audibles for calendar reminders.
You stated that DND was not enabled.
Therefore any settings in DND are irrelevant.
There is no Never in the Silence section of DND because it defeats the purpose of DND.
I think you are misunderstanding DND.
Essentially turning off DND is the Never Silence option.
Don't engage DND. Make sure that it's not turned on in Settings>Do Not Disturb. Make sure that both the manual and scheduled settings are off.
It's the earpiece, had to unplug it regardless if Bluetooth was on or off
