Why is there no 1280x1024 resolution option on my macbook pro when connecting to an external monitor via a KVM switch? None of the resolution options are useable.

What about without the KVM switch?
For some reason Apple dropped that resolution, along with the streched versions, when Lion was released. Don't know why. It seemed to be a popular choice,
Try this & see if it works out as a solution to your problem.
When I connect directly to the monitor with a displayport to HDMI cable it's fine.
When I say fine, it runs on the Mac-selected resolution of 1080p without the KVM switch - which is fine.
