I was streaming an episode of GOT online then suddenly iTunes opened behind my Safari screen and my volume went up by a notch. Could I have been hacked by someone or is it only a simple glitch of the system?

You probably accidentally bumped the built-in play button and/or the built-in volume up button. See the image below:

Also, in case you didn't know, whenever you press the Play/Pause button and are in a program that does not support OS X's built-in function keys, it will automatically launch iTunes.
It is very likely that you acidentally bumped it, as if a hacker did get into your computer, wouldn't they want to do more than just listen to your music?
