Rear camera on my iPad 3 will not focus.

I cannot get the rear camera on my iPad 4 (iPad with Retina) to autofocus. When trying to scan QR codes or iTunes gift card codes, the camera cannot focus on the given object. When i tap on the screen, it will not focus on the point I'm tapping. I have cleaned the lens. I have tried a factory reset with setting up as new through iTunes. I have cleaned the camera lens. I am at a loss and looking for any other suggestions. Thanks!

Since it's still occuring after a restore and set up as new via iTunes and it's only with the rear camera (im assuming the front is working fine), then you will need to get service for your iPad to fix the problem.
