What's the cause of not forwarding call on iPhone 5, reflection of "call failled "when making calls, even when phone is restarted/refreshed?

What's the problem/fault when iPhone 5 64GB fail to forward outgoing call, even when refreshed/restarted?

Any remedy please.

Are you asking about making a call, or forwarding a call? Those are two different things. It seems you are referring to a call failing, not forwarding.

Are you able to make a call at all? If not, then I would contact the carrier and ask them if there is something wrong with your account.
