Cursor changes appearance to hand cursor

Mac OS X 10.7.5

Insert cursor 'I' seems to be missing most of the time and replaced by 'hand cursor'.

If I enlarge my cursor via Universal Access>Mouse & Trackpad>Cursor Size it begins to behave inappropriately when typing in the browers (Safari 6.1.2).

What happens is when there should be an insert cursor, to insert text or edit something already typed, it instead shows a HAND cursor. Sometimes it will revert back to a insert cursor but not always and generally not after you've been typing something.

It does this on forums, on google docs.

I have high resolution dual 27" monitors (2560x1440) so having the slightly larger cursor helps locate it, otherwise the normal cursor 'hand' is maybe just 1/8" in width. TBH the hand still appears even if I reset the cursor back to normal size as in when I'm typing this message. Where's the 'insert cursor?' Any ideas?
