Make sure that the billing address that you entered in iTunes matches the billing address that you get on the statement exactly. No abbreviations if none appear on the billing statement ...things like Road v Rd., Street v St.

The zip code must match or country code, PO box all of those things. They must match exactly.

Make sure that the security code on the back of the card has been entered correctly. Check the expiration date as well.
