Cannot see calls while on a call

Over the past few days I have noticed that when I am on a call, I can't see who else is calling me (no caller ID) --I just hear the call coming in. When I pull it away from my ear, all I see are the call options, but not the caller ID. Also, when my call is over there is nothing in my "Recent" list showing me who was calling. Is there as fix for this? Thanks.
