unauthorized in app purchases refund

How do I get refunded for unauthorized in app purchases made by my grandson? I received an email telling me i was probably eligible but cannot navigate to where I fill out the forms to request the refund.

I have the same problem! I have a charge that exceeds the amount of the app and then an additional purchase that's don't find anywhere on the billing and I can't get the answer anywhere.
