iPhone sound problem HELP PLEASE

As I make a call on my iPhone 4, when speaking the sound does not go through to the other person I am speaking with. I can hear the voice of the person however they cannot hear anything from my phone. The only way that sound goes through to the other person is when i put the call on loudspeaker. Please help.
