iPhone 5 Battery life keeps changing when placed on charger

Got an iPhone 5 and have never had issues with it till now. One minute the phone will be fully charged then the battery indication quickly decreases and it'll say 38 percent and dies immediately. I'll put it back on the charger and it will say fully charged; the second I remove it immediately shuts off again. Please help!
