iPhone 5 Music App Glitch

When listening to music with my headphones whether the song is one downloaded to my phone from a personal cd or downloaded through iTunes purchase, the music randomly pauses and plays on it's own. Sometime it even does this when my headphones aren't in. It also goes to siri when my phone is locked and I'm on the music app. All of the problems seem to be revolving around the music app. It glitches, pauses and plays and goes to siri. It's a very frustrating issue and I need help finding a solution. Does anyone know how to fix this?
