iMac (27-inch): AMD Radeon 6970M Video Card Replacement Program

If you purchased AppleCare it would have been covered. It sounds like you do not have the 6970M graphics card installed and you chose to self insure which means once the 1 year warranty expired you accepted all responsiblities. AFAICT this is your responsiblity., sorry but that's how it goes sometimes.

Ok if that is the case I am not sure why you are even posting. Call AppleCare, advise of the problem and it should be repaired. Either that or take it to an Apple Store in your area, if you need to speak to the manager. Have all receipts and documentation with you and be polite!

Your last screenshot appears to show it having passed a video test. If it was the one relevant to the program, then unfortunately you'll have a little work to do in persuading Apple that the problem is one covered, rather than a 'similar-looking' one.

I think there was a similar question here before, where after contacting Apple customer service, the poster said that free repairs were provided after all.

Be patient, it will likely eventually crap out. I had problems a month ago, just before my Apple Care ran out along with an LCD backlight on the bottom left corner going out. The LCD problem was evident, but the video card passed their test. Now a month later, with much greater use of the computer, the graphics card failed brilliantly. BTW the program covers failure for FOUR years from a June 4, 2014 Apple Support document. Look here:

You are not contacting Apple here, we are all end users like yourself. Apple will replace the card with a replacement Radeon 6970M, one that has been upgraded so the same type of failure should not occur.

Well I had mine replaced and the replacement just failed. BOOM! Green stripes and then shutdown. For fecks sake!
