There may be a problem with the Gracenote naming system, especially

with older versions of iTunes; I'd looked into iTunes Support site but did

not see an exact match to this question in the form of an Article there.

    •iTunes - Apple Support:

    http://www.apple.com/support/itunes/

Seems the issue of correct title of music score and album gone missing

has occurred for  other users, at different times. Not sure if that is a matter

of Library file corruption or some other cause. Be careful if you attempt to

restore or rebuild the library, as that may damage the actual music files.

Do you backup your iTunes Library to an external drive? If you, you could

see if the missing song/album data re-appears in the backed-up archive.

To search  'gracenote missing song album titles' may yield helpful results...

At times, an internet search can show Support's articles faster than Support.

Such as this example, from google following a search using this...

    ' gracenote itunes ' -- https://www.google.com/#q=gracenote+itunes

    How iTunes remembers audio CDs

Importing songs from DVD should be simple, but the names may have to

be found online if they've gone missing in iTunes. Hopefully that still works.

(The iTunes Radio, failed last month, came back a few days ago in my G4s)

PS: Gracenote web site has a Support page, w/ link to Consumers

    http://www.gracenote.com/support/

    http://support.gracenote.com/pkb_Home?l=en_US&c=Public%3AConsumers

(you can submit your own question, but may come back saying contact Apple)

    Good luck!
