Q: Google Survey

I keep getting this survey asking about Chrome. I know this is not something Google has distributed and I can't get it to go away. Any suggestions besides fowarding the info to Google which I have done?

Thanks

iMac, OS X Mavericks (10.9.2)

Is this an email survey?  Add a rule in mail that just deletes it.  Or is this survey some sort of a pop-up?  Then there's something installed (Google Earth?) that's probably involved.

It may not hurt to do some house cleaning in Chrome > Settings > Show Advanced Settings > Privacy
