If you have a iPhone that was setup for work there may be a configuration profile on it that requires an encrypted backup, see the first article below for more information.
If that isn't it, make sure you have deselected the Encrypt iPhone backup option on the iPhone Summary screen in iTunes as shown in the second article.

Resolution
One of these situations may apply:
- Backup encryption was enabled, but you do not know the password
- An administrator installed a profile on your device that requires backup encryption
When backup encryption is first enabled, iTunes asks to enter a password and then creates a new encrypted backup for the device. iTunes will continue making encrypted backups until you deselect the Encrypt backup checkbox in the iTunes Device Summary screen. You need the backup password to disable backup encryption or to restore from an encrypted backup. The setting to encrypt backups is stored on the device.

When your backup is encrypted, you'll need to enter the password when enabling or disabling encryption or when restoring from this backup.

I have religiously done my homework - trying every possible password that I have used that I can remember, reading countless articles both on this site as well as on the web - all to no avail. I was hoping that deleting all backups and starting from 'scratch' would remove the password, but that doesn't seem to be the case.
If I were to be so crazy as to do a factory restore and start from scratch, will iTunes revert back to still be looking for the password that I cannot remember?
