On the home screen you put your finger on the app till it wiggles then drag it on top of another app. Once the folder is formed tap the home button to stop the wiggling.

To create a folder in iOS, hold finger against app #1 till it wiggles, then drag it on top of app #2 and release. New folder is created, both apps deposited inside and you get a chance to change the suggested default name for it. Once created, you can drag additional apps into it in the same manner, there is no limit to how many it can hold (will create extra "pages" of the contents as needed). Note, however, that folder cannot be nested inside one another.
