How do you permanently delete music?

I have followed these instructions but every time I open Itunes the music I deleted reappears. This is happening on a new machine, fresh OS install and fresh Itunes install. Any idea as to why?

Try Hide and unhide iTunes Store, App Store, or iBooks Store purchases - Apple Support.

Are you using Apple Music or iTunes Match? If you are not, then you cannot delete the tunes from iCloud, you can only Hide them:

What I want to do is completely delete my icloud music library so it no longer exists. The library is made up of some music purchased through itunes and some ripped from my CD collection. Is there a way to reset or completely wipe the icloud music library or will I have to delete the account and start over? I am using Apple Music.

You should be able to delete anything that is in your library that you don't want. When deleting past purchases you should be asked if you want to hide as well or just remove the download. You should be able to delete content that shows with a cloud, again hiding purchases or permanently removing songs that were originally ripped and matched, or have been acquired via Apple Music. Either way you should be able to clear entries by selecting and deleting.

I select all then hit the delete key, I am then asked if I am sure if I want to delete this song from icloud music library, I click delete song. If I close itunes or click update my icloud music library the song reappears. Just tried deleting it from my apple music app on my phone, its gone from my library. I wish there was a quicker way lol. I appreciate everyone's help thank you. Now if only I can select multiple songs in the app.

Hmm. You can try the options here, but as an archived document they might not apply any more: If your music library shows incorrect details with iTunes Match or Apple Music, and you previously cancelled an iTunes M…. Otherwise you may need to contact iTunes Store support and get them to reset the library from their end.

1. At the bottom of the page, click Reset for the option to Reset iCloud Music Library.
2. When you’re asked to delete all of the music and playlists stored in your iCloud Music Library, check “I understand” and click Reset.

Unfortunately this is not available for me in my account. The reset option at the bottom indicates it only resets the warnings.

I suspected as much. I have iTunes Match rather than Apple Music, but I don't see that option any more either. Try step 4. in that document, otherwise reach out to Apple Support through the Get Support link on this page: Apple Music - Official Apple Support.
