If i unlock my iPhone 5 to use another carrier does this mean i have to delete everything off of it and start off as a new iphone/ restore from backup?

If you read the Article you would have seen that to complete the Unlock you need to Restore the Device using iTunes and after the Successful unlock you will be able to Restore from the Backup that was made Before the Unlock proceedure was commenced, should you wish to do so.
Probably not. When I unlocked my old iPhone 3G, nothing was lost. But it is still a very good idea to back up your phone completely (via iTunes, since apparently iCloud backups may not include some data and passwords) before you make any major change.
If possible, buy a cheap prepaid SIM card from someone other than your current carrier, and try it before you need the phone unlocked (probably for a trip). Occasionally the unlock does not work, and you may need to get it fixed. Keep your receipt for any fee you pay for unlocking so you won't have to pay again (I speak from experience).
