Volume controls only control headphone and not speakers

When I use the volume controls in settings or on the side of the iPad, only the headphone volume changes. How do I adjust the speaker volume instead of the headphone volume?

Does the speaker work at all? Are you getting sound through it? If the speaker is working, but the volume control doesn't work, try resetting the iPad.

Reset the iPad by holding down on the sleep and home buttons at the same time for about 10-15 seconds until the Apple Logo appears - ignore the red slider if it appears on the screen - let go of the buttons. Let the iPad start up.

If that fails, provide some detailed information about the problem if there is anything more that you can share... does it happen in all apps? Is it random? etc. etc....
