Why do songs randomly separate from an album? How can I merge them back into the album?

When I upload songs into my library either by adding files from the music folder on my Windows 7 64bit pc OR by inserting an audio CD, I have found that songs from the album randomly separate in my music library. Not sure why since the artist and album title still match - but just want to be able to get them to merge back as part of the album. Other pain point is that when they separate , they don't show the artwork?
