I have given my old iPhone 4S to a relative. It is not possible to revert to factory settings as the old phone asks for my Apple ID under an old email address as username. The original password which matched this name does not work. How do I therefore take it back to factory settings?

You need to recover use of the account and enter the Apple ID and password because of this:

You need to temporarily change your current AppleID back to old AppleID (do not send a verification email), delete the iCloud account on the old iPhone,then you can change to your AppleID back to the current one you are using.
