Creating a form for iPad/iPhone to send (via .csv?) to email account

I am trying to show my business the value of iPad / mobile devices in every day business activities. We have a nice little risk tool which manages standard risk registers and I'd like to demonstrate the value of using mobile devices to capture data (i.e "receive ") from mobile devices.

I thought a simple start would be to create a form I could set up on key user's iPads which would allow the data entry of risk detail (title, description, rating etc...) with a button to email the form to a server based email address - we've set up the email address to recognize these inputs and enter them to the internal tool.

What app can I use to create the form ready for iPad which then has the ability to send the form outputs (as .PDF / .CSV, .XLS whatever) to an email address?

Ive tried apps like Form Entry (and tried using the Form Entry Server) but with no luck.

Any ideas? Remember, at the moment, it's just one way from teh mobile to an email account - if we move to wanting to publish data from the server to iPads later, I will deal with that then...

with a button to email the form to a server based email address -

This is a mistake. There is no guarantee the the user will have an email client set up.

You should send the data to a form server which then emails you the results. Unfortunately, I cannot find a free form server to email.

I found some documentation.

Roll your own example here:

Your talking a fair amount of software development here. You should consider hiring a consultant to set this up.

Post back and I can provide an example form using cgiemail
