Q: Failed to store token in keychain

Mac - OSX 10.9.4 (13E28)

I'm getting persistent keychain requests to log in to allow access for various programs to my keychain.

If I try and start facetime, I put my password in, it accepts it, then immediately I get a system prompt window asking for password again. I put this in, but system refuses it. I then see in console that Keychain has been refused access to store the password in keychain.

This keychain issue is driving me mad! any experts shed any light, appears to have only started since last update.
