If I borrow a film from iTunes, do the 48hrs in which I can watch it start directly after the download or is there more time?

You have 30 days from when you rent the film - but once you start watching the film (even if only for a few seconds) you then have 48 hours (24 if you are in the US) before it expires and is deleted.
