I purchased an iPhone 4 for my son. He was initially linked to my iCloud account, but now we can't seem to get my Apple ID off as the default account for iCloud and Messaging and FaceTime. How can we get his Apple account as the default account on the iPhone?

Settings>Store>tap on your AppleID. Log out and log in with your son's Apple account info.
Unfortunately that didn't work. When I opened Settings and selected iTunes and App Store, my son was already logged in under his own Apple ID. The iCloud account remains under my Apple user ID.
Go to Settings > iCloud. Tap on 'Delete Account' at the bottom.
Go to Settings > FaceTime. Tap on the Apple ID and sign out.
Go to Settings > Messages > Send and Receive. Tap on the Apple ID and sign out.
Go to Settings > iTunes & App Store. Tap on the Apple ID and sign out.
Go to Settings > Game Center. Tap on the Apple ID and sign out.
Now go back to all those places and sign in with the desired Apple ID.
