To add to your config, I am using an SMIME key to sign my mail and get many mails that are also SMIME signed, which may be contributing to the problem. I am running a MacBook Air 2014 with the same software and Mail.app.
The specific error message is: "mdimport wants to use your confidential information stored in "Key from www.digicert.com" in your keychain. Do you want to allow access to this item?"
