Do you have a CDMA or GSM phone? Note the following quotation from iPhone: Understanding phone features
CDMA networks may not be able to add, swap, or merge calls in certain situations. See theiPhone user guide for more details.
The following is one quotation from the user guide and there may be other information there:
Note: With CDMA, you can’t switch between calls if the second call was outgoing, but you can merge the calls. You can’t merge calls if the second call was incoming. If you end the second call or the merged call, both calls are terminated.
