Q: how to get safari to remember login and password after I said do not remember

I had this problem last week and solved it with a Keychain Reset. Note that it does create an all new keychain, so make sure you have your other passwords recorded somewhere.

Awesome!  Spent hours trying to use other suggestions...yours solved it immediately!

Open Safari Preferences, switch to Password tab, you'll see a line which notes your site followed by "never saved" marker. Just remove it, then it will ask you again when you back to that site and enter your login info.

I'm on an iPhone 4s running iOS 9.1 and I can't seem to solve this issue. I have Keychain turned on and it's saving the password but it's not populating the username when I get to the website (www.vetcompanion.com) so I have to type it in each time. Does anyone have any other thoughts for me? Thanks,
