This is sooo frustrating.... have talked to all levels of support at AT&T and nobody can fix this. Have reset phone to factory defaults and no help! Since you are having the same problem with Verizon, it is clearly a phone issue. Just wondering if it is a hardware defect or software issue. I have a feeling it is hardware related!
I am glad to hear others are having similar problems!

Call Verizon and ask them to "Re-provision your account", and to make sure HD voice is enabled.
while your phone is on. Leave it on for about 30 minutes, depends how busy they are.
Turn off LTE.
Reset Network Setting.
It will reboot on it's own.
reboot again with power and home button..
go the Settings->Cellular->Enable LTE and select voice & data.
wait 5 minutes and reboot.
now lte work for me

Thanks for all your reply, first of all. I went to AT&T and they checked my account and the tech said everything is fine and I should be receiving LTE signal. He thinks that there is something wrong with the phone and suggested that I should go back to Apple. Before I left, he changed my SIM card just to make sure there's nothing wrong on their side. After changing, same issue I could not connect to LTE, only 4G. We also tried network reset after changing SIM card. I made appointment to Apple and have to come back home because my appointment time is 3 more hours. As soon as I came home, I noticed that LTE is finally working. I did a speed test and indeed I was getting LTE speed 20Mbps down. I canceled my appointment at Apple store because my phone is perfectly working now.
So, the thing that fixed my phone is by changing the SIM card and do a network reset on your phone. Wait for a few minutes and the LTE signal will turn on. Make sure you're in an area that can access LTE signal.

it was the reset on their part that did it.

I had the same problem. Upgraded from a 4s to a 6 and got a new SIM card. Try resetting the network settings under Settings->General->Reset; it will restart automatically. If you don't have LTE a few minutes after it restarts, call your provider. I have AT&T; they had to send a network reset from their end to get LTE to connect. (Be patient, but firm with them. They had a whole checklist of things they had to go through before they would try the reset, but IT WORKED.)

Sep 22, 2014 12:40 PM in response to drlynnby fanblade,I had this same issue with AT&T. I upgraded from an iPhone 4S to the new iPhone 6 and it would only connect to 4G, not LTE. Additionally, the data connection would often drop out entirely, which could be remedied by cycling airplane mode. I tried resetting network settings and rebooting the phone several times, but nothing helped.
Swapping the SIM card fixed the issue on my phone. I got a new SIM from an AT&T retail store. I spent TWO HOURS in online tech support chat with an AT&T representative before they finally agreed to let me swap SIMs. I powered off the phone, put in the new SIM, powered it on, and I immediately had LTE connectivity.

Sep 23, 2014 4:34 PM in response to Lady Engineerby jrturner007,No, I had it escalated to the network team in Texas and have been on the phone with them -- they are starting to hear that this is a known problem and more than likely it's not a "phone" problem, but a problem on their end (since numerous customers are starting to report this problem )
I'm still stuck on 3G -- I do have the number to their network guy
I tried your sequence and it didn't work for me BUT I didn't know about Verizon having to re-send the LTE setup files to my phone

Sep 23, 2014 4:41 PM in response to jrturner007by Lady Engineer,Yes, you have to have the phone on, the key is keeping the phone on for 30 minutes while they send it. Their systems are backed up.
After 30 minutes, that, reset networking only, it will re-boot, and then to setting, turn on LTE and select voice & date, did my brother's today and it worked. Everyone i spoke to did and it worked.
Ask them to "provision" your account. that should do it. that sends all your features. If it don't work, there backed up again.
Also make sure they see you iPhone 6 and not your previous phone.

The rep disabled that feature on my account and reset my device. While my reception meter is at two bars, I am able to make calls over LTE and browse the web relatively quickly.
