The problem is the volume is entirely too low whether you crank it all the way up. I would hope im bright enough to turn the volume up before logging onto an Apple support forum.
Kmitchum 1 is right, try to hit the volume up during call.....it works for me.
In call volume can only be adjusted during call, try it folks rather thn running around.

Plug in your headphones
Have someone call you
turn up the volume during the call
Unplug headphones and hang up
Have them call you back and
Viola!
This fixed my issue. Hope it helps someone.

Apparently there was an ivisable barrier of lent on the phone speaker. rubbed a q-tip of isopropyl alcohol on it, then dried it off with the dry end of a qtip. did it a couple times. My volume is back to normal. Tried everything else, and never worked. cleaning the speaker with a Q tip and isopropyl alcohol fixed it. ...Suggested solution from DavidPeru. Worked great.

This fixed my 6+. I used a Alcohol wipe. Had to turn volume down some after cleaning.

FINALY i tried something very silly and it worked.. just use a cotton-bud and rub it on the speaker area to clean it.. and Booooooom it worked..

AT&T is in the process of improving their voice calls over LTE in my area and my calls from iPhone 6 Plus to iPhone ^ were constantly dropping on the same network, with 4-5 bars. I made a call to my service provider and he actually disabled the voice call over LTE feature on my phone until the updates on their side are completed. BUUUUTTT, he didn't tell me anything about adjusting settings on my phone. Fast forward to now when I've been having various call issues including hearing an echo of myself when dialing out to non-AT&T mobile devices, and I knew it had something to do with that feature the tech specialist switched.
Anywhoot, I found this thread and then I found your answer and I really wrote all of this just to say THANK YOU SO MUCH!!!

Aug 17, 2015 12:42 PM in response to McGIOby Londononion,OK, so I had the same issue. It randomly fixed when I went to sound settings and switched off the vibrate on ring and vibrate on silent. I called voicemail to check and then switched the options back on and now all working fine.

I think the "linkage" between ringer volume and in call speaker volume is clearly a software issue.
