Health is supported on the iPhone 4s.

Should it still fail to function, the next step would be to restore your iPhone.

I don't think you're alone in having issues with the Health App. I added a number of sources and I still can't get it to display anything consistently. Sometimes the charts on the dashboard display with data, sometimes they don't display at all, mostly they display "No Data". I add charts to the dashboard and they randomly reset to not display on the dashboard. Trying to "show all data" shows a spinning wheel with no data displayed.
The app is currently taking 659mb of space on my iPhone 6, but for the most part it just does not display any of that data. I don't think I am going to restore my iphone until this thing gets fixed. It's (for me at least) completely useless.
