Unable to install OS X Yosemite on my Mac

I've downloaded the OS upgrade to my Mac and all seemed OK until installation. Now, I can't get beyond a screen that announces "OS X could not be installed on your computer. File system verify or repair failed. Quit the installer to restart your computer and try again." Anyone have any ideas?User uploaded file

Update: I've carefully followed Leroy's instructions but without success so far. I didn't get the "Repair Permissions" option or the "Repair Disk" either initially. So, following the onscreen instruction, I selected "Verify Disk" which then enabled "Repair Disk" option. That failed so I got instructions to backu, reformat and then restore. All seems odd after not having any issues prior to downloading Yosemite! I'm now in the process of doing that and will advise on (hopefull) success, or otherwise, afterward.

I Have the same issue with a different error msg. Gets to aboutb15 mins install then fails.

ive tried repairing disk (judging by the log the installer does this anyway during installation).

ive even tried booting from USB key with the installer on a separate volume, this produces the

same error. My files are still on the disk, It just won't accept Yosemite ans Mavericks seems to not be there

aanymore to roll back to.
