Well, I'm finding how to transfer the call from the Mac to the iPhone but not from the iPhone to the Mac.
OS X Yosemite: Use your Mac to make and receive phone calls
Transfer the call to your iPhone: Unlock your iPhone, then tap the “Touch to return to call” banner at the top of the screen on your iPhone.

Looks like it can't be done (and, for what matters) it also seems that it's
not possible to initiate conference calls on Yosemite.

Well, that's discouraging. I just had a situation in which I was listening to a conference call muted on my Mac, switched the call to my iPhone to speak during the conference call, then discovered that I could not switch back. Really bad there's no two-way functionality here.
